How to login:

Your username is your MasterID as found on your first aid certificates and other documents.

Your password is your date of birth in the following format: m/d/yyyy. For example if your birthday was the 12th of March, 1960. Your password is "3/12/1960". Please do not enter leading zero's for months or days.

Include forward slashes in your password.
